Anderson Summary: A select few drugs are used to treat or prevent asthma. This article identifies the major drugs in a short paragraph, when they are used, and what their purpose is and action expected. Of the 22 million people in the United States who have asthma (2008 data), 5,000 died last year due to the disease or complications resulting from it. "Everyone who wheezes does NOT have asthma." This famous and correct statement was made years ago by a renowned physician. It is repeated to every medical student, respiratory therapist, and nurse today. One of the first steps in tagging someone "asthmatic" is to rule out every other possibility. There are detailed questionnaires, extensive physical exams, blood tests, breathing tests, skin tests, etc. which go into confirming the diagnosis. Asthma is a serious condition. People (children and adults) die from severe asthmatic attacks each year. The National Institutes of Health (NIH) has a special panel of expert doctors who have written guidelines and protocols to help patients, parents, and health professionals. The most important outcome of asthma control is not the drugs used, but rather the overall return to an acceptable quality of life to all concerned. With that said, let me address the current list of drugs used and based on when in the disease cycle they are used. Asthma is composed of inflammation (swelling) and obstruction of the airways (breathing tubes). Drugs used to treat it occur at different times in the process and do very different things. Anti-Inflammatory Drug. Therefore the first medication is an anti-inflammatory drug, a steroid. The steroid reduces the swelling and decreases the white blood cell activity and its accumulation in the airways. Rescue Breather Puffer. The next drug recommended for all asthmatics is a "rescue breather aerosol". (Albuterol or Proventil Inhalers) This is a puffer which contains a short-acting bronchodilator. The bronchodilator relaxes the smooth muscles surrounding the airways (especially the small distant ones) allowing them to open (dilate). Every diagnosed asthmatic must have a rescue breather readily available…end of story. It takes a five to twenty minutes for this medicine to hit peak effect, therefore keeping calm until it is fully functional is vital. Too many patients take it incorrectly and then take multiple doses...not good. Albuterol with Terbutaline: Short acting drugs. These are the typical rescue agents in asthma that are introduced into the lungs via aerosol (mist) inhalation powered by an small medical air compressor or medical oxygen (if available) . The onset of action is very rapid (2-5 minutes) and it lasts for about 2-6 hours. It is a good drug for acute asthma attacks. If the asthmatic is not relieved quickly, call your doctor or emergency room for advice immediately. Longer-Acting Agents. Some patients benefit from drugs which are long-acting bronchodilators used as a preventive tool. These are a combination of drugs which do the same thing as other bronchodilators but are not as strong or do their work over a longer period of time. Salmeterol: This drug is not used for acute asthma attack because its onset of action is approximately 30 minutes, but if used, bronchodilation lasts for about 12 hours. It is more commonly used for long term control in combination with anti-inflammatory asthma drugs. It is not considered an anti-inflammatory drug because it is still a bronchodilator (beta-2 agonist). Ipratropum : This drug is usually combined with Albuterol or Salmeterol as Combivent. It blocks the receptors in the small and medium airway muscles to reduce the bronchospasm and accumulation of mucus in the respiratory system. Other Drugs Cromolyn: Cromolyn is an anti-inflammatory drug that works as a mast cell stabilizer to keep them from rupturing. Mast cells exist in target organs throughout the body. They contain histamine. When the cells are ruptured, histamine and other mediators of inflammation are released causing more significant inflammatory reactions. Zafirlukast
and Montelukast (Singulair): Leukotriene receptor
antagonists, which means they tie up the receptor site on the
bronchial muscle so bronchospasm does not occur due to the presence
of these blood-borne agents. Used in moderate to severe asthma, but
are not as effective as corticosteroids. They can be toxic to the
